  1. Provide a brief description of the case. Address as many of these questions as possible:
         a. How was this incident reported?
         b. Did an adverse event result from the error?
         c. Was the physician contacted about the error/adverse
             event?
         d. Did the error occur under the laboratory's authority?
         e. Which lab functions were responsible for the error?
         f. Was the error preventable?
         g. Was the error cognitive, or non-cognitive?
        
  2. Remove all PHI (Protected Health Information) from your case description. Cases are reviewed by editorial staff and may be altered to ensure anonymity or to enhance instructional value.
