Dr. Astion is Associate Professor and Director of Reference Laboratory Services in the University of Washington Department of Laboratory Medicine. He is actively involved in a number of projects related to patient safety and the clinical laboratory. He also develops training and competency assessment software for clinical laboratories and laboratory training programs. To date, he has developed more than 20 software course titles used by over 2,000 institutions worldwide.

Ms. Ahlin is the Senior Vice President and Director of Quality and Compliance for ARUP Laboratories. She is an adjunct assistant professor in the University of Utah Department of Pathology. She has over 30 years experience in clinical microbiology and laboratory administration. She has lead the introduction and implementation of quality initiatives at ARUP Laboratories as well as lectured and published on the topic. She is also a member of the IQLM Technical Partnership Advisory Council and the CLMA Quality Advisory Council.

Dr. Chiozza is a researcher at the Department of Pediatrics of Padua Hospital and is responsible for all research granted by the Veneto Region on error prevention in the Pediatric Department. She is a trained Pediatrician, Urologist, Researcher, Consultant and is responsible for the Quality Service of the University Hospital of Padova in Italy. Dr. Chiozza has written more than 70 scientific articles published in international journals and is currently a member of the Bocconi School of Management, SIQuAS - VRQ (Italian Society for Quality in Health Care) and ISQUA (International Society for Quality in Health Care).

Dr. Edelstein is Director of the Clinical Microbiology Laboratory at the University of Pennsylvania Medical Center, an attending physician on the Infectious Diseases Consultation Service at the medical center, and Professor of Pathology and Laboratory Medicine at the University of Pennsylvania School of Medicine. He is board certified in Internal Medicine and Infectious Diseases, and is a Diplomate of the American Board of Medical Micrology and a Fellow of the American Academy of Microbiology. His major field of interest is Legionnaires' disease.

Dr. Hernandez is Assistant Professor of Laboratory Medicine at Mayo Clinic, Department of Laboratory Medicine and Pathology. He is the Chair of the Division of Clinical Core Laboratory Services and Chair of the Clinical Practice Committee in the Department of Laboratory Medicine and Pathology. Board-certified in Anatomic and Clinical Pathology, Dr. Hernandez is actively involved in many quality and safety initiatives in the laboratories at Mayo Clinic and serves on the Quality Oversight Committee.

Dr. Howerton is Chief of the Laboratory Practice Evaluation and Genomics Branch, Division of Laboratory Systems, Public Health Practice Program Office, at the U.S. Centers for Disease Control and Prevention (CDC). Her technical background is in clinical immunology, but more recently she has focused her efforts on laboratory quality assurance and quality systems development, working both at the CDC and in a large independent laboratory network. She is currently involved in CDC's efforts to develop the Institute for Quality in Laboratory Medicine (IQLM).

Dr. Jackson is Medical Director of Informatics at ARUP Laboratories and Adjunct Assistant Professor of Pathology at the University of Utah. His clinical activities involve promoting the appropriate ordering and interpretation of laboratory tests; his primary research interest is in assessing utilization of laboratory tests.

Dr. Pessin-Minsley is Assistant Professor in the Department of Pathology and Laboratory Medicine at the Weill Medical College of Cornell University. She is the Chief of Clinical Laboratory Services and Director of Point of Care at the New York Presbyterian Hospital, Weill Cornell Medical Center. Board certified in Clinical Pathology and Blood Banking/Transfusion Medicine, Dr. Pessin-Minsley has a major role in resident education. She is a member of the AACC Patient Safety Task Force, and has particular interest in Quality and Patient Safety in Point of Care Testing.

Mario Plebani is the Chair of the Department of Laboratory Medicine at the University-Hospital in in Padova as well as the Chair of the Center of Biomedical Reserch in Castelfranco Veneto. He is Past-President of the Italian Society of Clinical Biochemistry and Molecular Clinical Biology (SIBioC), President of the International Society of Enzymology, Referee of the ECM National Commission for continuing education in medicine and Expert of the Consiglio Superiore di Sanit. He has published 445 original papers in accredited journals and is a member of the editorial boards of "Clinica Chimica Acta," "Clinical Biochemistry," and "Clinical Chemistry and Laboratory Medicine." He is also Associate Editor of "CRC Clinical Laboratory Sciences" and "International Journal of Biological Markers."

Dr. Sandhaus is Associate Professor of Pathology at Case Western Reserve University School of Medicine, and Director of the Core Laboratory for Hematology at University Hospitals of Cleveland. Dr. Sandhaus recently obtained a Master's degree in Epidemiology and Biostatistics with concentration in Health Services Research. She is particularly interested in reducing laboratory errors through effective communication of laboratory results. Dr. Sandhaus has given many educational programs in hematopathology and laboratory hematology for the American Society of Clinical Pathologists, and has served on the Hematology and Clinical Microscopy Resource Committee of the College of American Pathologists. She is Director of Clinical Pathology Residency Training at University Hospitals of Cleveland.

Kaveh G. Shojania, M.D., is Assistant Professor of Medicine at the University of Ottawa and a Clinical Scientist at the Ottawa Health Research Institute. Dr. Shojania has developed several projects aimed at disseminating concepts of patient safety to general medical audiences, including "Quality Grand Rounds," published in Annals of Internal Medicine and WebM&M. Modeled on traditional Morbidity and Mortality rounds, the website presents anonymously submitted cases via accompanied by invited commentaries on a monthly basis. In early 2004, Dr. Shojania coauthored "Internal Bleeding: The Truth Behind America's Terrifying Epidemic of Medical Mistakes." The book has garnered accolades in multiple print and electronic media including The New York Times, Good Morning America, NPR, CNN's American Morning and Amazon.com.

Jane Vargas was the Laboratory Director at Community Hospital of the Monterey Peninsula, and currently serves as Quality and Outside Education Coordinator there. She established and still teaches the Phlebotomy Program at Monterey Peninsula College, and also currently teaches Customer Service and Phlebotomy for San Jose State University and Hartnell College. She develops Phlebotomy, MLT, and CLS curriculum, and consults with healthcare organizations in the areas of specimen collection, customer service, and quality improvement.

Dr. Weiss is Professor of Pathology at the University of Utah School of Medicine and President/Chief Operating Officer of the University's ARUP Laboratories. He is also Staff Hematopathologist with service responsibilities in the Hematologic Flow Cytometry Laboratory at ARUP and on the Hematolopathology Consultation Service. Dr. Weiss is board certified in Anatomic and Clinical Pathology, with subspecialty certification in Hematology and Medical Microbiology. His research interests are in laboratory management and administration, including the role of the laboratory in protecting patient safety. Dr. Weiss chairs the Laboratory Management Resource Council of the ASCP, serves on the Economic Affairs Committee of the CAP, and is a member of the Board of Trustees of the American Pathology Foundation and the American Clinical Laboratory Association.
